HOW TO ENROLL IN THE SVCS HOME CARE PROGRAM
At Seniors Passionate Care Services (SVCS), our home care program is designed to provide essential, personalized care services exclusively for adults age 65 and older in Broward County. We are committed to helping you live safely, comfortably, and independently.
We invite all eligible community members to learn more and get started today.
STEP 1: PRE-SCREENING
Begin by scheduling an appointment online to determine your eligibility for the SVCS home care program.
Our enrollment team will guide you through the process, answer your questions, and explain what to expect. If needed, we can also assist you with Medicaid enrollment.
STEP 2: IN-HOME VISIT
Next, we will meet with you and your family in the comfort of your home to review enrollment paperwork. During this visit, we will discuss your healthcare needs, social support, and overall care goals, ensuring we fully understand how to best serve you.
STEP 3: APPLICATION PROCESSING
Once your application is submitted, the enrollment process typically takes approximately 90 days. Our team will keep you informed every step of the way.
STEP 4: PERSONALIZED CARE PLAN
After enrollment, we will work closely with you and your family to develop a customized care plan tailored to your health, social, and quality-of-life goals.
