HOW TO ENROLL IN THE SVCS FHHA PROGRAM
A Family Home Health Aide (FHHA) program enables eligible family members or guardians to be trained, employed, and paid by a licensed agency to provide skilled personal care to a medically fragile child (under 21) at home.
Step-by-Step: How to Enroll in the Family Home Health Aide Program in Florida
1. Confirm the child’s eligibility
The child must be under 21, medically fragile, Medicaid eligible, receiving or eligible for Private Duty Nursing, and have physician-ordered PDN services in the plan of care. FHHA services are designed to support, not replace or duplicate, PDN services.
2. Contact Seniors Passionate Care Services
Call (877) 771-2128 and let the intake team know you are interested in the Family Home Health Aide Program.
3. Complete an initial screening
The agency will review the child’s Medicaid status, PDN authorization, current care needs, physician orders, and whether the family member or guardian may qualify to become the paid caregiver.
4. Submit required documents
Be prepared to provide the child’s Medicaid information, physician orders, plan of care, PDN authorization, caregiver identification, and any required background screening or employment documents.
5. Enroll the family caregiver for training
The family member or guardian must complete the required Florida FHHA training program before providing services. Florida guidance describes this as an AHCA-approved training program, with 86 hours identified in current provider updates.
6. Complete competency validation
After training, the caregiver must demonstrate competency in approved care tasks such as personal care support, mobility assistance, medication-related support, seizure monitoring, G-tube support, tracheostomy support, or other approved tasks based on the child’s plan of care.
7. Become employed by the licensed home health agency
The family caregiver must be hired by a participating licensed home health agency before Florida Medicaid can reimburse FHHA services.
8. Coordinate FHHA hours with PDN services
FHHA services may be authorized up to 8 hours per day and must not overlap with or duplicate authorized Private Duty Nursing hours.
9. Begin care under RN supervision
Services must be supervised by a non-related registered nurse employed by the home health agency. The RN helps ensure care is safe, appropriate, and follows the child’s plan of care.
10. Maintain ongoing compliance
The caregiver must follow agency policies, complete required documentation, maintain training requirements, and participate in ongoing supervision and care coordination.
