WHAT DOES PRIMARY CARE SERVICE INCLUDE?
Primary care service includes annual physicals, preventive screenings, chronic condition management, prescriptions, lab orders, recommended cancer screenings, referrals to in- person specialists, and sick visits.
Annual physicals are recommended for all adults. They establish your health baseline, monitor changes, and help you be proactive about your wellness. Primary care service also includes AHCA 5000–3008 physical examination for Medicaid Long-term Care Patients, Emergency Medical Condition (EMC) service for chiropractors and therapists, and Pre-employment physical examination to ensure new hires are physically or mentally capable of doing a job.
PREVENTIVE SCREENINGS
Preventive screenings are usually part of annual exams. They commonly involve in-office blood draws, but some preventive screenings require X-rays or other forms of lab testing. Screenings can detect early-stage health problems before they become an emergency, including conditions such as Type 2 diabetes, high cholesterol, high blood pressure, and some types of cancer.
CHRONIC CONDITION MANAGEMENT
About 6 in 10 Americans have a chronic condition such as diabetes, heart disease, or cancer, and the number of people with chronic conditions is expected to increase by more than 1% every year. Chronic condition management provides customized treatment, such as weight loss management, lifestyle counseling, and medication management.
